When your child needs orthodontic treatment, the qualifications of the provider matter as much as the treatment itself. At Pediatric Dentistry of San Jose, our pediatric dental team works alongside a dedicated orthodontist to deliver coordinated care from first checkup through final retainer. Having both services under one roof means your child’s dental history, growth patterns, and treatment goals are managed by a team that already knows them.
Dr. Zachary Hollander earned his DDS from Columbia University and completed his orthodontic training at UCLA, where he also earned a master’s degree in Oral Biology. He is a Diplomate of the American Board of Orthodontics and has been published in the American Journal of Orthodontics and Dentofacial Orthopedics for his research on the Maxillary Skeletal Expander. His professional affiliations include the American Association of Orthodontists, the Pacific Coast Society of Orthodontists, and the College of Diplomates of the American Board of Orthodontics.
Clear aligner therapy has changed how teens experience orthodontic treatment. These custom-made, nearly invisible trays address many common alignment issues without the look of traditional braces. The American Association of Orthodontists notes that most patients wear clear aligners for 20 to 22 hours per day, removing them only for meals and brushing.
Clear aligners work well for spacing issues, mild to moderate crowding, and certain bite problems. Many teens prefer them because they are removable, easier to clean around, and far less noticeable than metal brackets. Today’s orthodontic appliances are more comfortable than ever, and while some mild soreness is normal during the first few days after new aligner trays, it typically fades quickly. Dr. Hollander evaluates each patient individually to determine whether aligners are the right fit based on the complexity of the case and the patient’s ability to wear them consistently.
Traditional metal braces remain the most versatile option for children and teens with complex alignment needs. Today’s brackets are smaller, more comfortable, and more efficient than previous generations. They can address severe crowding, significant bite problems, and cases that require precise control over tooth movement that aligners may not achieve on their own.
Many children enjoy selecting colorful elastic bands to personalize their braces, which makes the process more engaging. Most children adapt quickly and can continue eating most of their favorite foods, though sticky, hard, and crunchy items should be avoided during treatment. Our team provides brushing and flossing guidance specific to braces so your child can keep their teeth and gums healthy throughout the process. Traditional braces are also typically the most cost-effective orthodontic option and provide the most predictable results for complex cases. For patients who need additional support, palatal expanders and space maintainers can be used alongside braces to guide jaw growth and hold room for permanent teeth.
The American Association of Orthodontists recommends that children have their first orthodontic evaluation by age seven. At this age, enough permanent teeth are present for Dr. Hollander to identify developing issues with jaw growth, bite alignment, and tooth spacing. An early evaluation does not always mean immediate treatment. In many cases, our team monitors your child’s development and acts when the timing will produce the best outcome.
When early intervention is recommended, Phase 1 treatment typically takes place between ages six and ten and focuses on guiding jaw growth and correcting structural issues before all permanent teeth have arrived. This can reduce the scope of Phase 2 treatment during the teenage years or, in some cases, eliminate the need for it entirely. For complex cases that may eventually require surgical coordination, our team works with trusted oral surgery providers to plan the best path forward.
Your child’s first orthodontic visit includes a thorough evaluation of tooth alignment, bite relationship, jaw development, and facial symmetry. Dr. Hollander uses digital imaging to assess what is happening below the surface and discusses findings with you in clear, straightforward terms. If treatment is recommended, you will receive a detailed plan covering the type of appliance, estimated timeline, and what to expect at each stage.
Treatment duration varies by case. Braces typically require 12 to 24 months for most children and teens, while clear aligner timelines depend on the complexity of the alignment issue. Throughout treatment, regular follow-up appointments allow Dr. Hollander to track progress, make adjustments, and address any questions. After active treatment is complete, a retainer holds teeth in their new positions and protects the results long-term.
