How to Make an Appointment
To schedule an appointment, please contact our office at (408) 259-1130 or fill out our online appointment request form.
Our friendly staff will assist you in finding a convenient time for your child’s visit.
What to Bring with You to the First Appointment
For your child’s first appointment, please bring the following items:
- Your child’s dental insurance card, if applicable
- A list of any medications your child is currently taking
- Any relevant medical history information
- Your completed patient forms
Patient Forms
We accept most all PPO dental insurance plans, and our staff is here to assist you with submitting claims. Please download and complete our patient forms prior to your child’s appointment to expedite the check-in process.
